After School Karate Program

I run an after school karate program and we have the students sign a release and waiver of liability form. Currently we have the students either mail the form in or turn them into their instructor.

Does this form need to be filled out, signed and turned in or can I have them fill out this form online and submit it?

It doesn't matter where the form is printed. However, if the students are minors, there's a good chance that the document will not have any binding effect.

If minors you need to have the appropriate parental or guardian signing the waiver on behalf of the child.

If the person is an adult you don't have the same problem.

Also, there have been some changes in liability for coaches and instructors that you may want to check out.

It doesn't matter if the form is filled out through a forms program (e.g. Adobe) or printed on their home computer or whether its mailed in instead of handed in. What you should prefer is a "wet blue ink" signature instead of just a click-through web consent or an electronic signature. That is a good balance between making it easy on them and making it stick when you need it to.
