terminate a sole proprietorship

Hello.

My business started as a Sole Proprietorship in Santa Clara, Santa Clara County and later the business moved to another county and formed as an LLC. Now, I want to terminate the Sole Proprietorship in Santa Clara. I paid business tax and registered the fictitious business name in Santa Clara. My question is this, is there any specific steps or forms I need to fill out to terminate Sole Proprietorship?

You should take reasonable steps to notify (and pay off) any creditors that might be affected by the change of business ownership. Creditors would include any employees who might have vested rights such as vacation.

No. You can file a notice to abandon your fictitious business name or you can just let it expire. Or you can transfer the name to your new business if it will have any operations in Santa Clara County. Notify the business tax people that you have moved your business.
