Will a simple consent to be videotaped form signed before the start
of a Baseball year allow someone to videotape and sell DVDs of that game
for profit? Would doing it as a "fundraiser" be different if the "fundraiser" was legit?

The form would have to include consent to the use you want to put the video to.
