Legal Question in Civil Rights Law in California

my ex and i have a child. I filled out the paperwork for the dr with my and my wifes information on some of the pages. my ex went into the dr and was given a copy of the paperwork i filled out including my wife and my birthday address and social security number. is this illegal. I dont want to her to have this informaion what can i do?

This is not a civil rights issue since government and fundamental rights were not involved. In California both parents are allowed access to all the information in a child's medical file.
