How can I file a motion to dismiss for lack of Jurisdiction if I live out of state?

Everything depends on the court you are in.

Note that motions to dismiss usually most be drafted by hand -- it's not simply a matter of filling out forms. You should also know that being out of stated does not automatically mean the court has no jurisdiction over you. I would consult an attorney about the jurisdictional issue.
