Good-afternoon, please advice what proper legal form i need to use in order to submit a response for a Summon i received - for $4,601.13 left in debt. Thank you.

There are a couple of other things you might do before filing an answer if the complaint warrants a demurrer or motion to strike. But you can download the Answer-Contract from http://www.courts.ca.gov/documents/pldc010.pdf . You would complete it on your computer, print it, sign it, have a friend or relative serve it and sign a proof of service, file it with the court, and mail a copy to the plaintiff's lawyer. Make sure to investigate affirmative defenses and include them in the answer. If you don't do that, you might lose the right to raise them as defenses later on.
