How do you fill out a notice of completion for new home construction after it is completed.

In reply to your inquiry, let me offer the following:

Per Cal. Civil Code � 3093, in general, you sign and verify a statement giving:

a. the date of completion (Must be actual completion - everything done, and within the ten days preceding the date the Notice is recorded);

b. Owner's name and address;

c. Nature of Owner's interest or estate in the property, e.g., owner of the fee, leasehold for x number of years;

d. Site address and, preferably, legal description of the property;

e. The name of the contractor who contracted directly with the Owner and a general description of the work performed by that contractor.

The Notice must be recorded in the County Recorder's Office not later than 10 days after actual completion, otherwise, its ineffective to shorten the time in which subcontractors or other lower-tier claimant can record mechanics lien claims.
