Can a Doctor start charging interest on an outstanding balance without telling me in some way? There's no notice of them saying that they would charge interest, on any invoice, just started adding interest to my outstanding balance

There probably was some fine print in something you signed when you first started going to that doctor. The simple thing to do is ask them what the basis is and to show you what you signed. If they can't show anything you signed agreeing to interest on unpaid balances, they can't charge it, although if they sue you for the underlying fees, they would be entitled to interest.
