Most of the first page is for the plaintiff to complete. You want to provide dates when you'll complete your discovery -- answers to your interrogatories and document demands, and when you will not be available for trial (vacation, weddings, etc.). You tell the court whether you agree to mediation or arbitration. You also tell the court what the case is about from your perspective. You're also supposed to meet and confer by phone with plaintiff's attorney, and tell the court that you have done so. That's usually what the court's concerned about.
