Claim of exemption financial statement form

Submitted claim of exemption form to levy

Officer. Went to court and Judge told me

the Financial statement form was

incomplete. Said I needed to submit a

amended Financial statement form. He

then set another court date. Do I need to

send the amended statement to the levy

officer or do I just take it to court with me?

Re: Claim of exemption financial statement form

You should file the original with the court before the hearing and mail copy to the judgment creditor. Someont other than you must to the mailing and must sign a proof of service. The proof of service must be filed with the court as well.
