Debt- Collection Summons

I received a Summons (California) from an Attorney representing a Collection Agency on a outstanding debt. Please advise on th following:

1) What legal form do I use to respond to the Summons?

2) If I can't afford the an attorney or filing fees, what forms should be completed as well as a waiver for all applicable fees?

3) If I want all prior documents proving that I owe the amounts stated on the Summons, what forms are needed to be completed as well?

Lastly, do the courts draw up the above forms and forward to the Plaintiff's Attorney, or do I really need an attorney to file all of the above mentioned forms on my behalf?

1. Answer (and affirmative defenses) unless your filing a Motion or Demurrer. go here: http://www.courtinfo.ca.gov/cgi-bin/forms.cgi

2. There are forms for waiver of costs.

3. You will need to do a Request to Produce for which there is not a Court form. You can look in law library.

The Courts do not prepare the forms or serve them on the other side.
