what form do i need to fill out and submit to the court if i'm being sued

There are different ways to respond to a lawsuit: 1) answer; 2) demurrer, 3) motion to strike, 4) motion to quash. You might have defenses including the running of a statute of limitations. So, it's difficult to suggest a "form" without more information. You would both file with the court and have someone else mail a copy of your responsive pleading to the plaintiff's attorney.
