need a name or location of official letter

I just would like to know what the official letter is that you send to credit agencies requesting information to what the exact detailed billing is on your credit report for the account in dispute, if they do not answer back within 30 days they have to report that the bill/bad credit is wrong. what is called or where i might find an example of the letter
