Legal Question in Credit and Debt Law in California

How do I respond to a summons from creditor? I have the answer sheet but it wants me to check boxes and explain what I don't agree to. I am not disputing any part of law suit I know I owe the money but we weren't able to set up payment arrangements with them because they were asking for 25% down and wouldn't work with me.
