I got served a summons regarding a credit card debt collection Rule 3740. What is the right legal form for me to use as a written response to court?

The language on the official summons form, that there might be some form you can use, is misleading. Merely filling out some court form might or might not help you, and might well hurt you. Most people who aren't lawyers don't know how to respond properly to a lawsuit. If you think you have a defense to the lawsuit, consult an attorney right away. Most credit card contracts have an attorney fees clause that will pay your attorney fees if you win the case.
