Legal Question in Credit and Debt Law in California

I need to sue an individual for breach of written, notorized loan. The amount is $15K, Pomona, Ca. What forms do I need to file. I would like a sheriff to deliver summons. Thank you for your time.

There are form summonses and complaints on the Judicial Council website. You also will have to file a civil case cover sheet and, for Los Angeles County, a civil case cover sheet addendum and statement of location.

You can attempt to be your own lawyer, but be forewarned that, invariably, a defense lawyer probably will make things difficult for you legally if you don't have your own legal advisor.
