verification of creditor status and debt

When a credit card creditor or collections agent makes

a demand for payment of a debt , what proof must be

provided :

1.regarding amount owed and how it was determined,

2.whether the claimee is actually the obligor of the debt

3. whether the creditor/claimant is the legal obligee on

the debt
