written notice
contract I signed for membership in gym states written notice to terminate membership-I gave notice in person and surrendered membership card-they say not acceptable 15 months later

Send the collection agency a copy of the written notice you gave the gym in accordance with the contract, and demand that they stop contacting you. Next time use certified mail RRR.
