what legal form do i need to file a written respose to a summons i've been served with

It depends. Please search the archives. The short answer is either:

1. A motion to quash;

2. A demurrer

3. An answer

It depends on the particular facts, you may want to respond with a Motion or demurrer. Or an "Answer" with "affirmative defenses" may be in order.
