Where can I find a notice of appeal form for the 9th Circuit Court of Appeals?

They are available at the district court clerk's office, though most attorneys would draft one instead (just a pleading paper titled Notice of Appeal... Plaintiff hereby appeals from the Judgment dated so-and-so.) Do not miss the deadline! If you are having this much trouble just filing a notice of appeal (there is much more work involved in getting an appeal heard) may I respectfully suggest you don't know what you are doing.
