Yes. Such waivers are often included in plea agreements. Sometimes there are ways around them, so even if you took a deal that includes a waiver you may still have options.

Yes - If you enter a plea of guilty. Yes - If you do not file a timely notice of intent to file an appeal and also if you do not follow through with the filing of the appeal in a timely manner. There are strict time limitations, so consult with an experienced criminal defense attorney as soon as possible!

There are lots of ways to give up your right to appeal. You can plead guilty. You can simply fail to file your notice of appeal within the legal time limit. Appeals are overrated, anyway. The place to fight your case is in the trial court.
