Client Declaration

Does anyone know where to access a sample of a declaration one would take from a client in preparation of filing a motion?

I, the undersigned, declare:

1. My name is [full name]. (I am [a plantiff/defendant/other capacity/etc.] in this matter.) The following is true of my personal knowledge and, if called to testify as a witness, I could and would testify competently to the following:

2. I was present at Harry's Bar at 123 Main Street in the city of Santa Fanta, California on January 1, 2009 and I witnessed the altercation between Joe Defendant and Sam Bouncer which is the subject of this case.

3. I was at the other end of the bar, a distance of approximately fifty feet from the altercation, and I did not see Joe Defendant bite off Sam Bouncer's nose.

4. However, I am informed and believe that Joe Defendant did, in fact, bite off Sam Bouncer's nose. I infer this to be true because, as Joe Defendant left the bar, I saw him spit it out.

I declare under penalty of perjury under the laws of the State of California that the foregoing is true and correct, except as to matters therein stated as upon information and belief, and as to all such matters I believe it to be true.
