CR-125/JV-525 or Subp-002

I am the victim in a criminal case. I want to do a debtor examination to enforce restitution and want to subpoena (Duces Tecum) the defendant. Which form should I file?

If you are talking about restitution as part of your criminal case (not as a separate civil proceeding), the prosecutor should do all the work in getting the witnesses for the restitution hearing. It is his/her job.
