What is the form I need to file for a motion for return of property?

There is not a judicial counsel form. You would have to file a Penal Code Section 1538.5 motion for the return of your property. Your local library should have a book with the format for the motion. A motion to suppress evidence under 1538.5 can be modified to request return of the property rather than suppressing it at trial.
