Many counties have their own PC 1203.4 (expungement) forms. You can certainly get one from the clerk's office at your local courthouse. Many other California Judicial Council forms are available at www.courtinfo.ca.gov/forms .

Contact the probation department in the county in which the offense occurred. The probation department should have the form and will process the expungement for a nominal fee.
