There aren't really forms necessary. If you tell your attorney you want a Marsden hearing, they're obligated to inform the court & you can have your hearing without filing anything. If your attorney doesn't let the court know, you can speak up and tell the judge yourself.

I'd discuss whatever dissatisfaction and issues you have with your attorney before taking the step of requesting a Marsden motion, though.

There are no forms. Just let the court know that you want to have your court-appointed lawyer removed. If still have questions about the process, feel free to email.
