petition for modificaton/termination of probation

I am trying to fill out a petition for modification of probation paper (pc 1203.3) but I am not sure what to put on item #4. It states : CASE CALENDARED FOR:____ at 8:00 A.M. in Dept___ for request of modification of sentence. Does this section have to be filled out by me or the clerk?If its me,what should I put here? ( this is the first paper I have ever turned into the court)

Call the clerk's office and ask if you need to reserve a date, then either reserve or pick a date at least 15 days out, 10 + 5 for mailing, someone else must serve a copy by mail on the DA's office and signs a proof of service which is attached to the last page of the petition.
