Well, if you had an attorney representing you on the matter contact the attorney and ask them for assistance. If you had a public defender representing you call them and they may be able to offer assistance. However you go about getting back in front of the judge you will need to have the Physician prepare a document to the court probably in the form of a declaration which provides under penalty of perjury that you are unable due to your heart condition to participant in the community service. You also need to contact the district attorney and let them know the situation as well. However, depending on the condition and the severity they may have you perform your community service in an office answering phones or something real basic like filing. It just depends on the county, the limits of your condition and what they have set up.
