Altered DMV Document

My arresting officer gave me my pink copy of the administrative per se suspension notice and no box except the ''surrendered license to officer'' box was checked. After the DMV hearing in which I was found to have refused the chemical test did I notice that the DMV's photostatic copy of the document entered into evidence had an ''X'' in the ''refused to submit to a chemical test'' box. What, if any, laws or statutes were violated by the officer or the DMV???

You do have 15 days from the date the decision was rendered to appeal this matter. The file in it's entirety gets sent to Sacramento and a decision will be made there. That is basically your only remedy.
