My Grandmother wants to make sure that her burial wishes are carried out after she dies. What kind of legal papers does she need to fill out?

Re: Burial Wishes
There is no document that can mandate burial wishes. Your grandmother can make her wishes known through an extra provision in the advance health care directive. Though this is not common or legally binding.
Her best bet is to pre-pay for her funeral or burial expenses now and that will ensure the greatest chance that her wishes will be completed.
The other thing your grandmother can do is prepare a letter of burial instructions to her loved ones -- and when she passes away, she can only trust that her loved ones will honor her wishes.
I hope this helps.
