How can I get a declaration of Incompetency form?

Not clear what you are asking about, but if you are talking about a Conservatorship you are probably referring to a "Capacity Declaration". This is a Judicial Council form that is completed by a physician and the physician, based on a recent examination, can indicate what deficits the Conservatee may have and if dementia powers are indicated. The form # is GC 335 and can be found on line.
