what information would i need to prove that my dad is incompetent?

Only a court can declare your father incompetent in a Conservatorship proceeding. Under some trusts and powers of attorney one or two letters (depending on what the documents say) from doctors under penalty of perjury (generally) stating that your father is incompetent.
