Legal Question in Family Law in California

I am 61 years old and was adopted at birth (May 1950). The 'legal papers' were signed about 2 months prior to my birth. I was born and the adoption finalized(?) in Humboldt County California. I have contacted the court in Humboldt County and they said that to unseal the adoption record I need to file a petition, but they would not provide the format.

I know the identity of my biological mother and she died in 2007. Both of my adoptive parents are deceased. For medical reasons I would like to determine the identity of my biological father.

Couls someone assist me with the correct format for the needed petition?

It appears that the larger Counties( San Francisco, Alameda, Sacramento) have local court forms which could be adapted for use in Humboldt County. There are also online resources and blog relating to unsealing adopting records, which should be of help. Last resort (sic), hire an attorney to do the work for you.
