My mother-in-law is adopting her own grandchild, and they told her to get a form which states that if anything were to happen to her such as she pass away or becomes to sick to care for her grandchild that she's adopting, that who does she want to give parental rights of that adopted child too. What form would she use to have in case something like that were to happen to her. Is it a living will trust or something like that. This is a form that she and the person whom she's is giving parental rights to are to sign and its states that if anything were to happen to her, that the other person has those rights. This is also so biological parent, knows that the child is to go to whom ever the adopted parent states whom she wants the child to go to. Basically so biological parent does not try to gain custody of child again. thank you.

A will is generally used to name the guardian of the child if the parents die. The agency handling the adoption may have a form that they want used. She should inquire of the person that asked her to prepare this form.
