Need advise on parental

I was with my exgrieldfrine for a while after we split up she move and now she came back just to let me know she put child support on me and i don't even know my kids they don't have the same last name as me. What can i do and what form should i use in court to be able to change my kids last name and be able to see my kids. At least once a month

You need to file a petition to establish paternity and custody. The other alternative is to demand a dna test for paternity.
