There is no judicial council form. You must prepare the notice of appeal on pleading paper. The court of appeals has some general instructions, however, most attorneys don't handle appeals and the issues that can be presented on appeal are limited. You cannot just appeal because you do not like the order or ruling. Seek the advice of an attorney who does appeals. I charge $500 to review a case to advise whether there are grounds for appeal.
