Appealing A Child Custody Order

My ex-girlfriend was granted custody of our three year old son while I was in jail. What form do I need to appeal this decision? Thank you. BV.

You probably do not want to appeal this decision. Instead, when you get out of jail, you can file and serve a form called a motion to modify child custody with the court. The court will set a date for mediation and a hearing.
