Legal Question in Family Law in California

My attorney completed an Attachment to Form FL341 for my holiday schedule under the Child Visitation section. I notice on Form FL300 there is a form FL341(C) listed under custody. It says this form is for a holiday schedule. Why would my attorney have added an attachment under visitation instead of FL341(c) under custody?
