Attorney not subbing out

My attorney of record has been jerking me around for over a week... says he gave his girlfriend the sub of attorney to mail to me but doesn't know if she did. He said it fax it and email a copy as well... despite daily calls + emails he still has not done so. He does not answer his telephone, does not return calls, does not respond to emails. He's been informed that my 'ex' who is out of state with my 16 year old has moved once again and refuses to give me her address, and is refusing to send the child for court-ordered visitation. He's been informed she may be a flight risk. I need new counsel, but how can I get this guy off the case without him subbing out? The previous judge (commissioner) Reva Goetz is no longer on the case... its now Judge Rudolph Diaz in Central Distrcit (Los Angeles). I need someone who knows this judge well, who's a tough-as-nails attorney. My 'ex' has violated numerous court orders.
