name change and abandonment

i have a 4 year old son thats i've raised by myself since he was 4mths. i had a restraining order on his father because he hit me and when i filed for custody wheni he was 1 yr he wanted visitation, i agreed to supervised visits he showed up 2-3 times an never again we never went back to court. now i would like to change his last name and remove him from the birth certificate is this possible????
