Name Change

I would like to change my name back to what it was before I got married, then divorced. Unfortunately, I am a starving graduate student and cannot afford the cost of $350.00 to do this. Can I file an application for waiver of court fees and costs or does this only apply to people who are being sued?

Go to Accesslaw on the internet. Select judicial council forms; scroll down to the bottom of that section to forms by category. Select the family law section. Then scroll down to form FL 395*. This form is an exparte application to restore former name. The court may charge filing fee for this form. In Santa Clara County the total filing fee would be about 52.50. If this is more than you can afford get the forms for the a fee waiver. Many counties have a family law clinic that gives free legal advice. This form should give you the result that you want.
