Name Change on Birth Certificate for Minor

My daughter was born in CA in 1994. We just had a paternity test and realized the identity of her biological father. We need to change her last name on her birth certificate, but we now live in PA. Where do I need to get the necessary forms from? Michelle

Re: Name Change on Birth Certificate for Minor

You should check with your local Department of State. You should be able to apply for a change of name if you have the original birth certificate.
