Name change on a birth cirt?

My daughter has gone by my maiden name and all her school documents, bank statements and social security card are with my maden name as well. How can I get her birth cirtificate to reflect the only name she has used since she was a little girl. Her deadbeat dad hasn't been in the picture for over a decade now, so that is not an issue.

You cannot change the name on the birth certificate. You can receive an order recognizing a change in name but it will not change the name on the certificate. Sounds like she is using your name on everything except the birth certificate so she should be recognized by your name. If you need any further information, feel free to contact me-Martin
