Name Change

When my daughter was born in 1989 the records clerk came in and asked me and my husband (boyfriend at the time) what name the baby was going to have, we told her that she was going to carry my boyfriend/husbands name. She then asked if we were married and I told her no, she continued to say that our daughter could not legally have my husband/boyfriends name if we were not married. I came to find out later this was untrue and it was her personal preference. Since then my daughter has had my maiden name. How do we change her name on her birth certificate?

you can file a petition for a name change in the superior court.

Forms for name changes are available at the courthouse, or they are available online at "www.courtinfo.ca.gov/cgi-bin/forms.cgi". I would suggest you consult an attorney to assure that the forms are filled out correctly, since name changes are relatively inexpensive where both parents are making the request.
