Legal Question in Family Law in California

name change

Hi I would like to change my daughters sir name. She has gone by the last name she uses now for the past eight years. But, its not official. I haven't seen or hered from the biological father in 12yrs. and don't have a clue how to find him. So, how do I go about filling out the form for a name change when it comes to the part about having to serve the biological parent notice of intended name change.

There is a procedure to obtain permission to serve the other parent by publishing notice in a newspaper if you can not locate him or her.
