name change via divorce

When I filed for divorce, I indicated that I wanted my maiden name back. I never changed it on my S.S#, driver's license, etc. It's been a few years since the divorce was final - what should I do now to take care of that?

File an "ex parte application for restoration of former name after entry of judgment."
