Legal Question in Family Law in California

name change

My divorce was final November 2002. I decided not to revert to my maiden name at the time because I still had children at home. I would now like to revert to my maiden name. How do I go about doing this? My divorce was granted in California but I now reside in Washington. Thanks for any help.

Re: name change

Form FM395 Ex Parte Name Change can be filed in the court that granted your divorce. You may be able to get a blank on line at Superior Court of California's site or call the family law clerk. There may be a small fee.
