Name change

I would like to change my last name to Luckett, which is my Father last name. The thing is he didn't know that Luckett was has last name,most of his life he only new--name removed--as a last name. he would also like to change his name to Luckett. Luckett is on his birth note but not on mine. what should I do?

You would have to file a petition for name change in the Superior court and follow the law applicable name change.
