Name Change

I have been married and divorced twice. My children are all from my first marriage and I would like to go back to that name to uncomplicate things. I am getting old and have a very serious illness. What do I need to do to take my first marriage name....or is it impossible to do?

Actually, it will be fairly simple. Get the form for restoration of previous name. The judge will sign and get about 5 certified copies, for Soc Sec, DMV, etc.
