name change of a minor when non custodial parent address unknown

What steps are required in notifying a non-custodial parent of a petition for name change if you do not know where they are living? I have full legal and phyiscal custody of my 11-year-old son and his father has not seen him since he was 18-months-old. Due to the school district requirements he is being forced to use his ''legal'' name at school and this is causing him embarrassment and discomfort since he has never gone by that name.
